What are your hours?

We are located across the street from Mercy South and McDonalds, about half a mile south of Rt. 173.


How do I get there?

From the North (Walworth Area):
Take Rt. 14 (Northwest Highway) south from Walworth until you arrive in Harvard. Continue south through the downtown area, crossing the bridge over the train tracks. We are located on the right side of the street, about half a mile south of Rt. 173.


From the South (Woodstock):
Take Rt. 14 north from Woodstock until you arrive in Harvard. We are located on the left side of the street, across from the McDonalds and Mercy South.


How do I make an appointment?

When you call, please have your doctor’s prescription for physical therapy and insurance information available. Physical Therapy prescriptions older than 30 days are not valid.

Call (847) 516-8095 Ext. 11 to schedule your first appointment. If you do not have a doctor’s prescription for physical therapy, we cannot schedule you for an initial evaluation, but we can schedule you for an Injury Assessment (link).




What should I bring to my first appointment?

For your first appointment bring: your doctor’s prescription for physical therapy, your insurance card, your date book for scheduling, and loose comfortable clothing.




How long will my appointment be?

Your first appointment will be about an hour long. After that, appointments are about 30-45 minutes long. Your frequency may vary from once a week to five days a week, but most patients are two to three times a week.




What should I wear?

Please wear comfortable, loose fitting clothing. It is important to wear something loose, so your therapist can examine and treat the injured area. Shorts and t-shirts are good for knee or back problems; tank tops or sleeveless shirts for shoulder or neck problems.




Will I always see the same physical therapist?

After your initial evaluation our front desk staff will try to schedule you with the same therapist for the remainder of your prescription. You may see another physical therapist occasionally if there are scheduling constraints.



How far in advance do I need to schedule my appointments?

Generally schedule at least 2 weeks in advance, or up to your next doctor's appointment.



What if I need to cancel or reschedule an appointment?

Please give us 24 hour notice of cancellation. If there's a cancellation within 24 hours, a $25 Cancellation or No-Show fee will be charged. For Workers Compensation No-Shows, your case manager will be notified.



What if I am late for my appointment?

In order to provide you with the best treatment possible, a late arrival will result in a shortened session time in order to remain on schedule for the next patient.



Can I bring my kids?
We have books, puzzles, games, and movies for your children to have fun during your treatment time. The clinic is a high traffic area and a dangerous place for little hands and running feet. In order to provide you with the best care, please remember that children must remain in the lobby during your appointment.


Will my physical therapist communicate with my doctor?

Yes, your physical therapist will reassess you prior to you next doctor’s appointment and write a progress note with recommendations. We will fax or deliver it to his or her office so that the doctor has it when you arrive. It is very important that you inform us of the date of your next doctor visit. If there are any unusual problems during your rehabilitation, the therapist will contact you doctor by phone to discuss his or her concerns.




What do I do after physical therapy?

On your last physical therapy appointment your therapist will finalize your home exercise program so you can continue your treatment at home. We also offer programs such as: Pilates, Massage Therapy, and the Fitness Program. These programs are offered to our patients to help maintain the gains you have made during your physical therapy treatments.



Will my insurance cover my physical therapy treatments?

Physical therapy is covered by most insurance plans. Please see our Insurance page (link) for a list of insurances we accept. We do bill your insurance company. However, if you have a copay, payment is due at the beginning of each treatment. Personal checks, Visa, Mastercard, and Discover are accepted. You are ultimately responsible for anything that your insurance does not cover.
